Photography and
testimonial consent.
Shann sometimes photographs an appointment, and clients sometimes offer a few kind words afterwards. Both are only ever used with your permission.
This form records what you are happy for us to use. You are free to say no to any part of it, and saying no makes no difference at all to your appointment or to the care you receive.
Tick only what you are comfortable with. Anything left unticked is a no, and we will treat it that way.
What this means

Consent is entirely voluntary, and you may withdraw it whenever you wish by writing to [email protected]. On withdrawal we will stop using the material and remove it from anything we control, normally within thirty days.
We should be honest that some material cannot be fully recalled once it is public. A social media post can be deleted, but we cannot recover a copy someone has already saved or shared, and printed material already distributed cannot be withdrawn.
We will not sell your photograph, pass it to another business for their own marketing, or alter it in a way that misrepresents you. We keep this record for as long as we use the material and for one year afterwards.
